Please complete this packet (fillable PDF) and submit it below along with an updated physical and immunization record.

Submit Camper Documents

All documents must be submitted at least 2 weeks prior to the start of camp.

Upload Physical (within 18 months of the camp week being attended) and Immunization Record

Upload NSTC forms: Emergency Contact, Health History, Camper Pick-Up, Sunscreen Policy, and Permission to Administer Epipen (if applicable)
